The links below will help providers to refer a patient and learn more about our services.

Thank you for your interest in having us treat your patient. We are accepting new patients with a referral from a medical doctor, doctor of osteopathy, nurse practitioner, physicians assistant, doctor of dentistry, doctor of podiatry, and a doctor of chiropractic.

We are an in-network provider for Medicare, Tricare, BCBS, United Healthcare, and DARS. 

A full evaluation report is sent to the referring provider  with every initial examination which includes a list of clinical test and findings and a diagnosis with further recommendations for imaging if necessary. Please download the physical therapy prescription form if needed, otherwise you may write a prescription for “physical therapy evaluate and treat” to allow us to initiate treatments.
