Gross Motor Scales- Reflexes
With hands around the trunk, hold child in standing position facing away. Tilt child slightly forward. Brush top of child's feet against edge of table, then hold child so feet are resting on table
(Lying on back, head toward examiner)
Turn child's face so left cheek is parallel to surface. Hold his or her head in that position for 3 seconds and observe child's reaction. Repeat procedure to right side.
Hold child suspended horizontally, stomach toward floor, side toward you with your hands under his or her chest and stomach.
(Either kneel on floor or stand facing table so when child is tilted forward, he or she can reach surface) Hold child in suspended horizontal position, stomach parallel to floor, buttocks toward you, then quickly tilt child's head toward the surface.
(Sitting, back toward you)
With hands at hips, support child in sitting position, then quickly tilt child 45 degrees to one side.
(Sitting, back toward you)
With hands at hip, support child in sitting position, then quickly tilt child 45 degrees forward

Sitting, pillows supporting hips. Attract child's attention to toy on a string suspended at midline 12 in. in front of child's chest.

Standing. Stand 3 feet from child. Say," I am going to move my arms and I want you to copy my movements." Do practice move (one not on test) to see if child understands. Do not use verbal cues. Present 6 positions one at a time at 10 second intervals.
Stand on 1 foot with hands on hips for 10 seconds, then on the other foot for 10 seconds. Say, "put your hands on your hips and stand on 1 foot and then the other like I did." Count seconds out loud to encourage child to balance longer.
Lying down on mat. Demonstrates sit-ups on mat. Please child in starting position on mat. Hold child's feet and say, "Do as many sit-ups as you can." Stop child after 30 seconds.
Lying down on mat. Demonstrate sit-ups on mat. Place child in starting position on mat. Hold child's feet and say, "do as many sit-ups as you can." Stop child after 30 seconds.
Lying face down on mat. Demonstrates 3 push-ups. Say, "Do as many push-ups as you can." Stop child after 20 seconds
Locomotion
Lying on back. Stimulates leg thrusts by holding child's feet and pushing them toward his or her body so knees are flexed, legs bent, and heels almost touching buttocks. Then pull child's feet out until legs are fully extended. Repeat motions. Let go of child's feet. Observe for more than 1 minute.
Lying on side, legs bend to maintain balance, examiner in back of child. Shake rattle 3 times behind child's back. Repeat procedure with child lying on opposite side.
Lying on back. Stimulates arms by bringing child's hands together at midchest with elbows bent. Then stretch arms out to sides until elbows are straight and hands touch surface. Repeat. Let go of child's hands. Observe for 1 minute.
Standing. Hold child in a standing position facing you with his or her feet resting on table or counter top. Observe leg position and whether child can bear weight for 3 seconds.
Lying on stomach, head turned to side, forearms resting on surface. Attract child's attention by shaking rattle 1in. above surface. Continue to shake rattle and move it 6in. above child's head.
Lying on back; feet toward you. Shake rattle 18 inches from child's nose and then move in to within 12 inch
Lying on stomach, chin and forearms resting on surface. Attract child's attention to toy on a string and then suspend it 12in. above child's face.
Lying on back, feet toward you. Shake rattle at midline 12 in. above child's face. Slowly move rattle in arc toward surface. Repeat procedure to other side.
Lying on stomach, head toward you. Attract child's attention to toy on a string that you dangle at midline 12 in from child's head. Observe child's arms and legs for 5 seconds.
Lying on back, bare feet. If child has socks on, remove them and then gently bend both legs toward child's face, wiggle and then release them.

Lying on back. Attract child's attention to toy by shaking it to side of child. Repeat procedure on opposite side.
Lying on stomach. Place toy 5 ft. in front of child. Say, "get the toy."
Lying on stomach. Sit 3 ft. in front of child. Hold your hands out to child and say, "come here."
Positioned on hands and knees. Place toy on floor 6 ft. in front of child. Say, "get the toy." Move toy back as child approaches.
Sitting. Sit beside child on floor. Say, "watch me." Demonstrate scooting by using your hands to propel your body forward on your buttocks to retrieve toy. Place toy 5 ft. in front of child. Say, "scoot like I did and get the toy."
Sitting. Place child in sitting position on floor. Attract child's attention to toy, then place it 2 ft. from child's right side. Say, "Turn and get the toy." Repeat procedure on opposite side.
Sitting next to stable object, such as chair or table. Attract child's attention to toy, then place it on edge of stable object, out of child's reach. Say, "get the toy."
Sitting on floor to one side of you. Sit with legs straight and knees touching. Attract child's attention to toy, then place toy on the other side of your legs so child will have to climb across your legs to retrieve it. Say, "get the toy."
Standing. Have child hold our index fingers. Stimulate bouncing by moving your hands up and down 2 times.
Standing next to low table. Place child in standing position at end of table. Place toy on opposite end of table. Say, "get the toy."
Place child in standing position with side next to stable object (chair or low table) for support. Place toy on floor in front of child. Say, "sit down and play with the toy."
With child facing you, support child in standing position with your hands around trunk. Say, "let's walk."
Place child in sitting position straddling one line of taped 3x3 ft. cross. Attract child's attention to toy, then place it on line 2 ft. behind child. Say, "turn and get the toy."
Place child in standing position next to stable object (chair or low table). Stand 4 ft. in front of child with your arms outstretched. Say, "come here."
Place child in standing position away from anything that can be used for support. Release your support of child. (Be ready to catch child if necessary.)
From in front, support child in standing position by holding 1 hand. Say,"Let's walk."
Sitting cross-legged on floor. Demonstrate standing up from sitting position. Place palms of hands on floow beside hips. Push down with hands, straighten arms, and shift weight to feet. Stand up without turning body more than 20 degrees to wither side. Say, "Get up like I did."
Standing. From the side, support child by holding 1 hand. Say, "Let's walk."
Standing. Hold toy 2 ft. in front of child. Say," Come get the toy." Move back as needed to keep toy just out of reach.
Standing. Place toy on floor 2 ft. in front of child. Say, "Get the toy and bring it to me."
Sitting on floor, facing stairs. Place toy on 3rd step. Say, "Get the toy." Move toy up as child gets closer. (Be prepared to catch child if necessary.)
Stand on 10 ft. in front of child, and hold your arms out. Say, "Come to me." [Record the time it takes to walk 10 ft. in next Item. It will be used later on in Item 41.]

Lying on back. Hold rattle 12 in. from child’s nose. Slowly move rattle in a 90 degree arc to one side (almost to the surface)> Return to midline and repeat procedure to other side.
Lying on back, head turned to side. Hold rattle 12 in. from child’s nose. Slowly move rattle in arc to midline. Repeat with child’s head turned to other side.
Sitting on lap, facing away from table. Using an upward movement, gently brush the back of hcild’s hand against table edge.
Lying on back. Hold rattle 12 in. from child’s nose. Slowly lower rattle to within 1 in of nose.
Lying on back. Hold child’s hands and wave them in front of face. If child’s arm are too short, turn child’s head to side and wave 1 hand.
Sitting on lap, facing table, examiner sits with side to table. Roll tennis ball on table from left to right. Say, “Watch the ball.”

Pick up another cube and carefully place it on top of the 2nd cube. Leave the tower standing and put 3 cubes in front of the child. Say, “You build a tower.”

Sit at a table with the child sitting on your lap facing the table. Make 2 vertical lines about 3 in. long on a piece of paper. Place a 2ndsheet of paper and a marker on the table in front of the child. Say, “Draw a line up and down like I did.”
Place a food pellet in the bottle and screw on the lid. Give the bottle to the child and Say, “Get the food.”
Place 10 cubes on the table. Say, “Watch me build a tall tower.” Pick up 1 cube and carefully place it on another cube so that the sides are aligned. Pick up another cube and carefully place it on the top of the 2nd cube. Continue stacking up the cubes until all 10 cubes are in the tower. Let the tower stand for a few seconds; then know it down. Put the 10 cubes in front of the child and say, “You build a tall tower.”
Holding your hands so the child can clearly see what you are doing, demonstrate cutting the edge of a piece of paper in 3 places with blunt scissors. Give the paper and scissors to the child. Say, “You cut the paper.”
Make 2 horizontal lines 3 in. long on a piece of paper. Place a 2nd sheet of paper and marker on the table in front of the child. Say, “Draw a line like I did.”
Holding your hands so the child can clearly see what you are doing, demonstrate stringing 2 beads onto a lace. Pick up a bead sothe hold is facing you. Show the hole to the child. Pick up the lace in your other hand and carefully thread it through the hold from front to back. Pull the bead halfway down the lace. Pick up another bead and string it in the same manner. Hand the lace with the 2 beads on it to the child. Put 4 additional beads on the table and say ,” String the beads like I did.”
Holding your hands so the child can clearly see what you are doing, demonstrate folding the paper in half. Leave the model folded and where the child can see it. Give the child the other half of the paper and say, “Fold it like mine.”
Place 4 cubes on the table. Holding your hands so the child can clearly see what you are doing, demonstrate building a train with 3 cubes aligned and 1 cube on top of the first cube. Push the train across the table making train sounds. Leave it in place where the child can see it. Place the 4 additional cubes on the table in fron of the child. Say, Make a train like mine.”
Holding your hands so the child can clearly see what you are doing, demonstrate stringing 2 beads onto a lace. Pick up a bead sothe hold is facing you. Show the hold to the child. Pick up the lace in your other hand and carefully thread it through the hole from the front to back. Pull the bead halfway down the lace. Pick up another bead and string it the same manner. Hand the lace with the 2 beads on it to the child. Put 4 additional beads on the table and say, “String all of these beads like I did.”
Place 5 cubes on the table. Say, “Watch me build a tower.” Pick up one cube and carefully place it on another cube sothat the sides are aligned. Pick up another cube and carefully place it on the top of the second cube. Continue stacking the cubes until all 5 cubes are in the tower. Let the tower stand for a few seconds, then knock it down. Put 10 cubes in front of the child and say, “Build a tall tower using as many blocks as you can.”
Place 3 cubes on the table. Holding your hand so the child can clearly see what you are doing, demonstrate building a 3 cube bridge as shown. Leave the bridge standing, Place 3 cubes on the table in front of the child. Say, “Build a bridge like mine.”
Place a piece of paper and a marker on the table in front of the child. Show the child that card with a circle on it and place it on the table about the paper. Say, “Draw a circle.”
Place 4 cubes on the table. Holding your hands so that the child can clearly see what you are doing, demonstrate building a pair of 2-cube towers in chich the towers touch each other to make a wall. Leave the wall standing. Place 4 additional cubes on the table in front of the child. Say, “Build a wall like mine.”
Holding your hands so the child can clearly see what you are doing, demonstrate cutting 1 of the 2 papers in half. Place the 2nd sheet of paper and blunt scissors on the table in front of the child. Say, “Cut the paper like I did.”
Show the child a strip with 6 holes, Say, “Watch me lace.” Holding your hands so the child can clearly see what you are doing, lace the stirring from the top down through the 1st hole, up through the 2nd hole, and down through the 3rd hole. Show the strip to the child then remove the lace. Give the strip and the lace to the child. Say, “You do it like I did.”
Sit at a table with the child sitting on your lap facing the table. Make 2 vertical lines about 3 in. long on a piece of paper. Place a 2ndsheet of paper and a marker on the table in front of the child. Say, “Draw a line up and down like I did.”
Give the child the paper and the scissors. As you run your index finger along the line, say, “Cut on the line.”
Place a piece of paper and a marker on the table in front of the child. Show the child the card with the cross on it and place it on the table above the paper. Say, “ Draw lines just like these that cross in the middle.”
Place a bottle (without a cap) and the 10 pellets on the table in front of the child. Say, “Put the food in the bottle as fast as you can. Put only 1 in at a time. “
Place the Paper in front of the child, with the line in a horizontal position. Give the child a marker and, as you move your finger along the length of the line, say, “Draw on this line. Try to stay right on the line.”
Place a piece of paper and a marker on the table in front of the child. Show the child that card with a square on it and place it on the table about the paper. Say, “Draw a square.”
Give the child the paper and the scissors. As you run your index finger around the circle, say, “Cut out the circle along the line.”
Place 6 cubes on table. Holding your hands so the child can clearly see what you are doing, demonstrate building steps with 3 cubes on the bottom, 2 on the next row, and 1 on top. Leave the steps standing briefly. Then disassemble and put the cubes in front of the child. Say, “Build the steps like I did.”
Place the paper on the table in front of the child. Give the child the marker and while painting first to 1 dot to the other dot, say, “Draw a straight line from 1 dot to the other dot.”
Give the child the paper and the scissors. As you run your index finger around the lines of the square, say, “Cut out the square along the lines. “
Place 6 cubes on the table. Holding your hands so the child can clearly see what you are doing, demonstrate building a 6- cube pyramid as illustrated. Leave the model standing. Put 6 additional cubes on the table in front of the child. Say, “Build one like mine.”
Show the child the piece of paper that has been folded with long sides together. Leave the folded paper on the table where the child can see it. Give the child the other sheet of paper. Say, “Fold your paper to look like this one.”
